Go online to the UMESD site and view the Assistive Technology link under Programs and Services. You can view the AT manual, which shows the flowchart on the right.
ASSISTIVE TECHNOLOGY PROCEDURES: Screening Form Fill out the screening form on the screening link. Fill out all appropriate content. STEP ONE
ASSISTIVE TECHNOLOGY PROCEDURES: STEP TWO Eligibility of a student for AT is assessed by the student’s team. STEP ONE
ASSISTIVE TECHNOLOGY PROCEDURES: If eligible and AT equipment needs to be assessed, go to the online UMESD AT Equipment Center or contact OTAP STEP THREE STEP TWO Fill out a check-out form online to reserve and have the item sent. Teacher may borrow an item for a 30 school day trial, rechecks are available, if not needed by another student. STEP ONE
