Bold Improvement Goals 1. • Embed the culture of continuous learning as part of this transformation • Use a consistent language that resonates with everyone in each setting across system i.e. from primary care, the community and to wider stakeholder e.g. council, third sector and other public service staff • Ensure that engagement outside of the hospital is at the core of the BIG programme and embedded in the way of working

Bold Improvement Goals 2. • Lead by example by demonstrating values through behaviours • Support ongoing measurement and sharing of outcomes to support performance management and delivery against the agreed initiatives. Therefore communicate incremental success as it happens ensuring that there is good evidence to support this • Follow through on promises

  7. Next Steps for C+V UHB BIG 3 • Three areas, Cataract, Occuloplastics and AMD • Safe and sustainable eye care services are a national priority for Wales. • Improvement for patients not to come to harm

  8. ECG Projects (C&VUHB) • Cataract Post-op discharge service into the community re-launch in April 2016. • GP awareness of Eye Health Examination Wales • Referral guidelines produced for all GP practices and GP OOHs • Primary Care Lead Optometrist attending Locality/Cluster meetings

  9. ECG Projects (C+V UHB) • Glaucoma follow up to the Community, launched August 2016 • Communication campaign for eye care ongoing for social media. • £15k training funds for Optometrists from WG, workforce opportunities • Visit to School of Optometry/Ophthalmology October 2016

  10. Conclusion • Change has been slow • Good solid foundation. • Eye Care Groups are pivotal to change. • Sustainable First Class service.
