Coverage amounts Employee • Basic Term Life and AD&D: $10,000 • Expanded Basic Term Life and AD&D: $1,000 increments to $40,000 • Supplemental Term Life and AD&D: $1,000 increments to $250,000 Spouse • $1,000 increments to $50,000 Child • $1,000 increments to $50,000 • Tax dependent children eligible from age 14 days to 26 years No dual coverage

October 2012 action items • One time opportunity to elect Expanded Basic Life and AD&D without Evidence of Insurability • One time opportunity to elect Spouse and/or Child coverage ($10,000) without Evidence of Insurability • Current Supplemental coverage amount carried over as-is unless employee elects a different amount online, or cancels coverage via Service Request Form
