On March 8th, 2012, Fizz Thompson, Executive Sponsor for the South Central Workstream, shared updates on the No Needless Ignorance initiative aimed at enhancing patient safety. Key goals include proactive identification and prevention of harm causes, creating an inspiring environment for NHS staff, and fostering open culture for reporting errors. Workstream Aims • To become more proactive in identification and prevention of causes of harm to patients • To create an environment that inspires all NHS staff to insist that care is as safe as possible • To ensure a more open culture in which errors or service failures are reported
