In 125 words or less, Heritage House is: …an assisted livingcommunity that brings together a wide range of services allowing vulnerable elders, who would be unable to live independently, to continue living in the community of their choice. Heritage House works with each resident to develop a service plan that meets their unique needs and that takes into account their life experiences, personal goals, strengths and challenges. Staff and residents work together to update the service plan as resident conditions change so that the services they utilize evolve with them. Our core values of Compassion, Dignity, Justice, Excellence and Integrity along with our promise of “Know me, care for me, ease my way” are reflected in the work that we do every day.

Rick • A longtime Market community member, Rick worked in the Market fresh out of high school. After a long career in the wholesale fish industry, Rick decided to pursue his dream of running a retail fish market at Pike Place. Unfortunately, his dream was short lived due to a stroke. • When there are Market folks who identify Pike Place Market as home, we want them to be able to stay here after they stop working
1955-2018 • Rick moved to Heritage House when he turned 62. This allowed him to cruise around the Market in his motorized wheelchair, preserve important relationships, stay connected to his community, and even find part time employment at Pike Place Chowder. • Rick’s memorial was held at The Market Commons.
Who are we? • Residents are: • People ranging in age from 62 to 101 • Medically stable & predictable (unless they are on Hospice services) • 91 % are Nursing Home Eligible as determined at the State level: Care needs in 2+ Activities of Daily Living; Qualified financially for Medicaid • Satisfaction is measured with annual satisfaction survey • Overall satisfaction of 79% • Staff are: • 30 Providence St. Joseph Health employees. • Average length of time with Providence is 3.8 years. • 3 people have been here over 14 years • 96% responded to the Caregiver Engagement Survey • 87% Sustainable Engagement with 8 out of 9 categories above the U.S Healthcare Norm
Goals for 2018 • Increase commitment to a resident-led and trauma-informed care model by fully embracing a culture of aging in which individual voices are heard and individual choices are respected. • Increase resident satisfaction. • Increase services to better support residents through end-of-life. People who are at end of life, have memory impairment or require behavioral support need additional resources to stay in the community.
Opportunities for 2018 • Heritage House serves a population of people that have outlived their financial resources and are funded thorough Medicaid. • The average daily rate for Medicaid covers only 72-75% of the total cost of care. • A majority of Heritage House residents have both physical and mental health needs. Because of the complexity of our residents need, Heritage House is often the only assisted living that will take them.

Purpose of Grant To support organizations addressing the needs of low-income members of the Market community by funding one-time mission focused needs outside of general operating expenses. This grant is open to any non-profit organization serving the Market community and awarded grant amounts will vary by project scope.
Purpose of Grant The Market community includes all people who live, work, conduct commerce or access services in and around the Market. The people of primary concern to the Foundation is supporting people within that broader Market community, which includes: • Low-income and homeless residents living in and around the Market • People who make their living through the Market (vendors, employees and business owners), and their families • People who access social services from the Foundation’s grantee organizations and through the Market Commons
Application and Selection Criteria Who Can Apply? • Nonprofit organizations submit a simple Form of Inquiry that includes an overview about the organization, project description, and stated grant request • Foundation staff will review Form of Inquiry to determine eligibility of organization to submit a full grant request proposal (Letter of Intent and Application) • Eligible organizations will be invited by Foundation staff to submit a full grant request proposal for funding consideration • Applications will be accepted year-round on a rolling basis and reviewed by CSC
Fund Development and Allocation • In any given year, the board will approve a budget for each grant program. • Rachel’s Reserve Grants will be awarded on a rolling basis three times per year - beginning in September through end of the fiscal year.
