Differential Diagnosis Patient THROMBOSED EXTERNAL HEMORRHOIDS Pain Sudden Straining Tender mass +\- fever +\- urinary retention • 1 week PTA painful sensation at the anal region after passing out hard stools. • Two days PTA, patient started to feel a soft mass over the R perianal region which was painful when touched. • Few hours PTA, patient started to experience painful sensation on the anal region which is aggravated in sitting position • DRE: erythematous, warm, and tender 5x4 cm mass at the R perianal region; digital exam cannot be tolerated by the patient

Differential Diagnosis Patient Anal Cancer Bleeding from the anus or rectum.Pain or pressure in the area around the anus.Itching or discharge from the anus.A lump near the anus.A change in bowel habits. • 1 week PTA painful sensation at the anal region after passing out hard stools. • Two days PTA, patient started to feel a soft mass over the R perianal region which was painful when touched. • Few hours PTA, patient started to experience painful sensation on the anal region which is aggravated in sitting position • DRE: erythematous, warm, and tender 5x4 cm mass at the R perianal region; digital exam cannot be tolerated by the patient
