NEW BLOOD LABELS at UCSD: Transition from Codabar to ISBT 128 blood labels Modified by UCSD Transfusion Service from: Americas Technical Advisory Group; ICBBA; www.iccbba.org NOTE: As of May 1, 2008, some blood products at UCSD Hillcrest and Thornton hospitals will be labeled differently, using ISBT128 barcoded labels. Specifically, all blood products from San Diego Blood Bank (which represent about 5% of UCSDs blood supply) will be ISBT 128 labeled as of May 1, 2008. Blood from the American Red Cross, which represents about 95% of UCSDs blood will continue to be labeled as is, until further notice. This is a modified version of the ISBT 128 Power Point presentation from ICCBBA. It contains the essential information and should be shared with all staff who will transfuse blood components. The key points to note are: 1. The new blood unit identifier (Unit ID) is 13 characters long (e.g., W1234 08 123456), and must not be truncated. Consequently, if a computer system or form is used to document transfused units, the "unit ID number" field must be able to hold at least 13 characters. 2. The blood product ID codes and other information are located in different areas of the label, as described here.     17. Important Notes: The goal of this presentation is, to alert you to the label changes and to assist you in identifying where the critical information will be found. This will avoid confusion, delays, and mistakes. As of May 1, 2008 some units will be ISBT labeled (ie the few that UCSD orders from SDBB). When Am Red Cross switches to ISBT labels (next year), all blood labels will be ISBT 128. This label change does not change any UCSD institutional policies for identifying patients and blood components at the bedside.
