Needs Assessment Tool (NAT) Webinar

This webinar training introduces the Needs Assessment Tool (NAT) and its benefits in improving care planning and consistency in the AAA network. It covers the new features of NAT and how it expands on the Level of Care Determination (LCD) to identify individual needs and develop personalized care plans.

  3. Changing for the Better • Respond to feedback from the AAA network • User friendly • Build consistency across the network • Level of Care Determination (LCD) focuses on the level of care • Needs Assessment Tool expands on information in the LCD • Identify individual needs • Develop care plan

Introduction to the NAT • Previously known as the Care Management Instrument (CMI) • Purpose is to gather information about the individual • New information is critical in development of a care plan • Will be used to determine individual’s service needs, strengths, resources, preferences, and goals • Information captured in the LCD will populate into the NAT using the SAMS copy feature (refer to SAMS instructions)

  5. Introduction to the NAT • NAT will populate from LOCA or CMI only if questions and response options are identical. • LCD training available: • www.ltltrainingpa.org

  6. Gathering Additional Information NAT gathers additional information necessary to complete a care plan. This information includes: • Diagnosis for Ears, Nose and Throat (ENT) and Eye Conditions • Nutrition and Nutritional Risks • Informal Supports • Protective Services • Physical Environment • Emergency • Financial Information

  7. What’s New in the NAT • Improve consistency with documentation • Drop down boxes for diagnoses • Fewer text entries • “Other” category • Note section for expanded documentation

What’s New in the NAT • SLUMS: Saint Louis University Mental Status • Replaces Short Portable Mental Status Questionnaire • Reasons for change: • Up to date screening tool • Mental status: score for normal, mild, moderate, severe dementia • Piloted in Cameron/Elk/McKean • More measurable year to year • No permission needed to utilize screening • No cost • Only 10 mins. to complete • Training available on LTLTI site • www.ltltrainingpa.org

  9. What’s New in the NAT • Functional Needs Score (FNS) • Formerly the FNM (Functional Needs Measurement) • Algorithm to assist with FNS • Prioritizes individuals for wait list placement • Results utilized by Department for quality improvement and other data collection

Completion of the NAT • Only complete when it is signed off by: • Care Manager • RN (when appropriate) • Supervisor - signature indicates: • Completion and consistency • Clear and supportive notes exist where required • All other signature and dates are completed • Required SAMS documentation (A&R and Journal entries) are completed THIS IS AN ELECTRONIC SIGNATURE Workers will need to be logged into SAMS with their credentials to complete this section
