  2. Question Sets What are Question Sets? Where can Questions Sets be used? How do you create a new Question Set?

  3. What are Question Sets? • Question Sets are groups of questions that can collect various Client information • Assessments, scales, eligibility information • Case Notes with specific prompts • Test Scores • Follow up progress • Each Question Set has a Question Code that identifies it

  4. Where can Questions be used? Central Intake Eligibility/Profile page Program Entry Program Assessments/Profile page Program Exit Follow-up after Client Exit

  5. Question Set Creation Organization Library: Question Template page A Question Code must be created before assigning to a Program or Service A Question Code may have multiple questions, each having multiple answers

Question Set FAQs • A Question Code is created for one Application • A Question Code name may be used only once • Names are Case Sensitive • The same Question Code may be used for multiple Activities and/or for Entry for multiple Programs • Each Question has a Consent Level assigned • Questions may be shared across Organizations or Groups • Questions can be restricted to specific Organizations or Groups • Some Questions sets are created by Adsystech and set as SYS, meaning they cannot be changed

Question FAQs - 2 • Questions may be copied from one set to another • These can change applications • Questions can use Rules so they appear only for certain Clients • Question order may be rearranged • Questions can be single or multiple selection • Multiple choice will have checkboxes available so User can check as many as apply • Questions in Central Intake can be grouped for filtering

Question FAQs - 3 • Answer format Normal means User can select or type in answer • Answer format can be Select Only (no fill in answers) • Answer format can be specific format • Dates, Social Security Numbers, Zip Code, Phone, etc. • Answers can have custom formats that you create • Useful for Account #s • Answers can be set to jump to specific questions • Each Answer can jump to a different question • Answers can be required to include Comment

  9. Viewing a Question Set • Select Application • Select Question Code • Questions for selected Question Code are displayed in Question grid • Answers for selected Question are displayed in Answer grid 1 2 3 4

  10. Create a new Question Set • Select Application • Create new Code • Create new Question and add information • Create additional Questions • Create Answers for each Question

  11. Creating Question Set Select Application Click NEW in Code container Enter Question Code name Enter Description Click SAVE

Question Fields Query table will be named QRY_”Code name” • Click NEW in Question container • Select Consent • Select Rule if needed • Determines which clients see question • Enter QRY Field Name if using for queries • This is Column Title • Select Multi Answer • Select Answer Format • Enter Group Name if needed • This determines Order alphabetically • Enter Question text

Question Checkboxes • Active = will show for any new client • Query = will be used for query in report • If checked, there MUST be a QRY Field Name entered • Required = User cannot save unless all required questions have answers • For Exit = will appear only for Exit questions, not Entry • Click SAVE

Copy From … • One or more questions can be copied from a different question code • Answers for each question will also be copied • Click COPY FROM In popup window: • Select Application • May be different Application • Select Question Code • Select one or more questions • Click COPY AND CLOSE • Questions and Answers will appear in grids

  15. Answers Select Question Click NEW in answer container Enter Answer text If answer should jump to different question, select that question in JUMP TO dropdown Check Comment Required checkbox if needed Click SAVE Repeat for additional Answers Repeat for additional Questions

Central Intake Questions 1 2 • Central Intake Program must be created first in Security Manager • Name must be Central (space) Intake • Question Code selection is what will be used • These Questions will appear on the Profile page in Central Intake • No Program Component or Activities need to be created • Question Code is created on Question Template page and selected here

  17. Program Entry & Exit Questions Question Code must be created first on Question Template page Select Program in Program grid Select Question Code in dropdown Click SAVE

  18. Activity Questions 1 2 • Activity Type = Profile • Select Question Code • Question Code is created on Question Template page and selected here

  19. Question Rules • Some Rules exist already • Click NEW • Enter Rule information • Enter Rule Name • Select Rule Type • Select Table • Select Identifier Field • Click GENERATE • Make selections on Common Query String popup window • Click ADD • Click SAVE AND CLOSE • Enter Comments • Click SAVE
