Family First Care Clinic now offers outpatient healthcare coverage for a small monthly price. You will continue to receive the same high quality care with unlimited outpatient visits. • Your Membership Will Cover: • *Acute Illnesses and Injuries • *Physical Examinations • *Chronic Illnesses/ Injuries • *Well/Sick Baby Care (greater than 2 months) • *Well/Sick Child Care • *Well/Sick Adult Care • *Well Sick Elderly car1e • *Immunizations • *Family Planning (BirthControl) • *Health Education (Individual and Group) • *Screening for Diseases • *Well Woman Examinations • Pap Smears Collection included • *Well Man Examinations • *Minor Surgery Procedures • *Diabetes Education/Management • *Referral Services to SpecialtyClinics • *Most Laboratory Test • *Most Medications administered in Clinic • While this policy will not cover Emergency Room visits and Hospital Stays, or treatment in other clinics, it will treat a wide variety of outpatient illnesses, injuries, labs, and minor surgery procedures in Family First Care Clinic. • Flexible Plans For Entire Family • Monthly Health Plan Membership Fee • Individual $75 • Two People $140 • Each Additional Person $30 Co-Pay of $20 dollars per visit.
