DOC Draft Process/Timeline: Processing SSI/MA Benefit Applications for Prison Inmates G O A L To obtain disability benefits upon release. • Collection of: • Green Packet • Notice Form DOC-___ • Medical Records (including community) • SSA-8000 Form Identification of potential disability candidates via referral. • Notification of potential • disability candidates: • Age 65+ upon release • “Open” Clinical Monitoring • DOC Contact DDB if Benefits • Determination Decision Not • Received Form DOC-15 prepared (sent to SSA). Phone call to SSA upon release. SSA-8000 Form prepared. • Parole Commission • Social Worker • Ph.D. • M.D. • Other IDENTIFICATION Notice COLLECTION & DISTRIBUTION (65+) DISTRIBUTION (under 65) COLLECTION (under 65) IDENTIFICATION R E L E A S E Days prior to release: 150 120 90 60 30 Days 14 REVIEW DETERMINATION FINALIZATION BENEFITS BEGIN Materials received at local SSA office.
