  1. Interconceptional Care MomsFirst

What is Interconceptional Care? • Often defined as an 18-24 month inter-pregnancy interval, it is the critical time to modify risks -- diseases, health behaviors, psychosocial risks, and environmental hazards -- that are associated with adverse outcomes for women and infants. • Interconception services and supports are also designed for high risk women who have experienced an adverse outcome in a previous pregnancy, and/or had a fetal loss, an infant born low or very low birth weight, or a neonatal death.

  3. MomsFirst Interconceptional Care Services • Assist with linkages to Medical Home • Ensure Medical Appointments are kept • Screen for Depression at six and twelve weeks postpartum • Provide education and resources relevant to case plan to improve self sufficiency • Vocational & Educational services, parenting, anger management, women’s health, etc. • Talk to mom about birth spacing (ideally 24 months)

  4. How can we improve our services? • In August 2009, all Healthy Start Projects attended an Interconceptional Care Learning Community Meeting.

  5. Interconceptional Care Learning Community (ICC-LC) • HRSA hosted an IIC-LC to focus on improving the health and well-being of women served by Healthy Start by advancing the quality and effectiveness of interconception care in their project area. The ICC-LC focuses on improving the health of high-risk women served by Healthy Start grantees through the implementation of evidence-based practices and innovative community-driven interventions, which are a hallmark of Healthy Start projects.

  6. Learning Collaborative Characteristics • Bringing together teams with a common goal (similar types of organizations) • Short term initiatives (6-15 months) • Learning Sessions (face-to face, learning from peers and experts) • Action Periods (in between learning sessions)

  7. Six categories to choose from: • Case Management • ICC Risk Screening • Family Planning • Primary Care • Maternal Depression • Healthy Weight

  8. Why Family Planning? • Family Planning contributes to the health of the woman as well as her children • Intendedness and wantedness of pregnancy • Optimal pregnancy spacing • Optimal health for mother and child • Optimal environment for child development

  9. Inter-relationship of Unintended Pregnancy and Family Planning • Effective contraception is important to avoid pregnancy • 40-60% of unintended pregnancies are due to incorrect or inconsistent birth control use • Unintended pregnancy rate could be cut in half if women were to use contraception correctly

  10. MomsFirst will… • Advance use of objective tools and data collection methods • Effectively collect information regarding Healthy Start participants’ actual use of family planning methods

  11. How will we do this? • Develop and implement a Reproductive Life Plan • Improve data system to effectively capture family planning methods • Train all outreach staff on family planning methods and new protocols

  12. What is a Reproductive Life Plan? • A set of personal goals about having or not having children • Participants set goals and think about how to reach them and how pregnancy and children play a role in those goals

  13. Sample Questions for Reproductive Life Plan • Did you (your partner) plan this pregnancy? • How old do you want to be when you have your next child? • How many children do you (your partner) want? • How far apart would you like you children to be? • What is your (your partner’s) plan to prevent another pregnancy?

  14. Additional Questions • What would you (your partner) do if you were to get pregnant again? • Are you comfortable talking to your partner(s) about condoms and STD’s? Why or Why not? • Where do you (your partner) see yourself in 5 years?

  15. How will this fit in with MomsFirst Services? • Enhanced communication about family planning with program participants • Goal Setting • Improving self-efficacy • Improved data collection around family planning

  16. Impacting Birth Outcomes By encouraging mom to take control of her health and her life, MomsFirst hopes to improve the birth outcome of her next pregnancy
