  2. 검사 장비 • 장비명 : ACL-TOP • Method : Turbidimetry =Cuvette에서 일어나는 clot형성에 따른 탁도 변화를 180도 각도의 투과 빛을 전자 감응부에서 감지하여 측정

  3. Prothrombin time (PT) • 혈장(Plasma)에 조직 thromboplastin과 Ca을 첨가하여 fibrin이 석출되는 시간을 측정. • 2,5,7,10 인자와 fibrinogen이 관여. • 외인성 및 공통성의 응고 과정을 검출.

  4. Prothrombin time 적응증 • 경구용 항응고제 복용 대상자(와파린) • 인공판막 치환자(기계판막) • 부정맥이 심하고 심장 기능이 떨어져 혈전의 위험이 큰 환자 • 혈전 병력이 있는 환자 • 기타 의사가 필요하다고 인정하는 환자

  5. INR:(International Nomalized Ratio) • INR(국제표준비율):경구용 항응고제 치료중인 환자의 관리지표 • INR=(Patient PT/Normal PT)ISI • PT시약내 thromboplastin의 성분에 따라 병원,국가마다 다른 PT sec의 민감도를 떠나 국제적으로 통용시킨 표준화 단위

  6. INR값 범위 • 심장에서 인공판막치환자 :2.5~3.5 • 그 외 질환(AMI,VHD)등 :2.0~3.0 • 심장질환이 아닌 정상인:0.83~1.2 * Percent는 정상인을 100으로 했을 때의 기준으로 sec와 반비례

  7. 결과 확인(1. PT>60sec) 1. 반응 end time이 측정되지 않음. 2. Error message: 그림 참조 3. Out of range : 그림 참조 * 처리방법: - 같은 검체로 Recheck후 결과 - 동일결과일 경우 : 외래인 경우가 많으므로 remark 처리 후 결과 전송 2, 3 1

  8. 결과확인(2. PT의 volume low) 1.반응 curve가 없다. 2.Error message: - Data curve min and max - Not in complex - Measured result failed * 처리방법: - Resampling 2 1

  10. 주의사항 • 냉장보관 시약 - APTT-SS, CaCl2 (액상) - PT Recombiplastin (분말) - 사용 시 실온에서 30분간 rolling • 제조방법 - PT Recombiplastin (Pipette 20mL)
