  2. Objectives • Welcome • Review Goals of Beacon IC3 Project • Update on current Utah Diabetes Practice Recommendations • Train on The Improvement Model • Develop a Team Improvement Aim • Update on the state Clinical Health Information Exchange • Evaluate the session

  3. IC3 Vision Prove the hypothesis that quality of healthcare can be improved while decreasing costs when providers are enabled through a foundation of health information technology used meaningfully and exchanged appropriately.

  5. Clinical Intervention • Excellent patient outcomes will be achieved when clinical processes and health IT are streamlined • Connectivity will improve our care and outcomes • Patient-engagement will be fostered by technology

  6. Who we are • 27 independent primary care clinics recruited, 1 podiatrist (MD, DO, PA, NP, DPM) • 10 University of Utah Community Clinics • Selected Intermountain Primary Care clinics • Using Electronic Health Records • Intend Meaningful Use attainment

  7. Second Quarter UCIT 2011 initiative • Summit • Tooele • Optometrists/Ophthalmologists/Podiatrists (referrals)
