Transforming Families, Young People and Children’s Services:Our Service Development Initiative Programme Deliverables New Service Model Configured around Neighbourhoods • That integrates mental and physical health and embeds prevention and health promotion through clinical pathways. • That provides earlier and timelier identification, assessment and intervention at the least intensive level through a consultative team approach. New processes & information technology solutions • That improve access for Children, Young People and Families through a single front door (unified access system). • To share information effectively, reduce duplication and integrate records to build a comprehensive picture of individuals and their family. Enhanced Health Visiting Workforce • With a year on year increase in the number of qualified Health Visitors in practice providing the Universal Healthy Child Programme. • Equipped with additional skills and knowledge integral to the consultative model of care. Development of Integrated Youth Pathways • For young people aged 14 – 25 years with mental and physical health needs, or Looked After Children (LAC) Development of Emergency Pathways SDI Timeline Phase 1 Current State Assessment: Complete by 2nd November 2012 Phase 2 Identification of Challenges and Areas Requiring Change: Complete by 31st January 2013 Phase 3 Development of Options for New Model (and agreement of preferred model): Complete by 31st March 2013 Implementation of New Model and Removal of 13/14 and 14/15 CIP: Complete by 1st October 2013 07/01/2013 – Version 3
