  5. Preadmission - Nursing • Invite carer to attend with patient • General overview / physical presentation • Baseline observations, ECG, medications • Check HQ and clarify points • Education • Brochures

  6. Preadmission (Cont) • Active listening. • Conversation. • “When I was in hospital last time….” • “My friend had…..” • Interview with carer to discuss home situation. • Understand process and involvement.

  8. Referral to Hospital In The Home • HITH nurse will meet patient in preadmission • HITH responsibilities • Contact numbers • Selection criteria • Obtain consent • Complete home visiting staff risk assessment form

  9. Discharge Planning • Carer to pick up • Responsibilities at home • Support for medication • Access to bathroom

  10. Carer Information Responsibilities • Organise household • Pick up & stay with patient for 24 hours • Assist with medication • Assist with hygiene • Seek medical assistance if necessary

  11. Day of Procedure - education • Arrival time • Fasting instructions • Preparation for theatre • Anaesthetic preparation • Recovery room • Day surgery clinical pathway

  12. Discharge Expectations – • Time for discharge • Discomfort and management • Nausea and management • Discharge criteria met HITH visit prior to discharge
