Health Center HIV Screening Algorithm Lab Test Lab HIV Test (Western Blot) Negative Positive Inform patient at next visit Give “Negative” handout, if desired Review risks, if appropriate Discuss “window period” No further testing • Schedule follow-up appointment with patient. At follow-up appointment: • Counsel patient • Draw CD4 & Viral load (if appropriate) • Introduce patient to HIV care manager or other team member, OR • Ensure referral to HIV care • Contact DIS (if applicable) • Complete “Reactive Tracking Sheet” Indeterminate Repeat Western Blot at follow-up visit Created by NACHC for use in community health centers, December 2012
