  1. Health Center Patient Visit Flow Process with Routine HIV Screening, Lab Test Dental Model Front desk greets patient and determines whether they are an appt. or walk-in. Walk-ins are offered a same day appt. if available (then proceed as an appointed patient). Front desk registers the patient, verifying insurance and demographics. Co-pay collected, if appropriate. Encounter form is printed, and together with the ROUTINE TESTING FLOW SHEET, placed in bin for pick-up by Dental Hygienist or Tech. Front Desk Waiting Room Dental Hygienist or Tech escorts patient to dental operatory. During intake Hygienist/Tech offers HIV SCREENING WITH LAB TEST. If accepted, Dental Hygienist or Tech notes lab order on flow sheet. (In states where patients are required to ‘opt-in’ to an HIV test, the patient will need to sign a consent form at this time). Dentist performs exam; orders necessary follow-up visits and/or referrals. Dentist writes lab order. Patient discharged; Hygienist/Tech places the Routine Screening Flow Sheet in project folder. Dental Operatory Laboratory Clinical Coordinator or other designated staff member is called to escort patient to the primary care center for a blood draw. Patient carries lab request to the lab for blood draw. Patient takes encounter form and proceeds to check out area for scheduling of next visit.
