System Generated User Accounts If an email address is provided in step 1 or 2, and a user account that contains that email address does not exist, an account is created automatically using the email address provided EX: user ID: christinec@dnr.state.la.us password: christinec@dnr.state.la.us1 An applicant that did not create a user account can use this log in information to view/modify applications filed by a third party

Necessary Information – Application Form • Applicant/Agent information • Landowner/Adjacent Landowner names and mailing addresses (attach additional sheet(s) if necessary) • Lat/Long of Project (attach additional sheet(s) if more than one point) • Excavation/Fill Volumes and Types of Materials (native material, sand bags, etc.) • Equipment Used (marsh buggy, shallow draft vessels, pipe lay barge, etc.)

  5. Necessary Information - Plats • POB, POE and all Points of Infection • Length and Diameter of Pipeline • Method of Installation (trench, push/pull on surface, directionally bore, etc.) • Depth of Installation • Pipeline Crossing and Tie-in Details (larger excavation area, bracing details and materials used, etc), Bore entry/exit location and size of required workspaces • Access Route, Water Depth, Vessel Draft
