Wessex HIEC vision The Wessex HIEC will create a network of organisations in Hampshire, the Isle of Wight, South Wiltshire and South Dorset to collaborate on: transforming services, working practices and care pathways in our region promoting independent living for people with long term conditions delivering care closer to home and reduce demand on acute services improving the health and wellbeing of the population we serve

Initial Initiatives Telecare programme: Long term conditions Supporting care closer to home and managing acute conditions in the community. Service transformation programme: Equip health and social care staff to work in new ways, Shifting care into community settings Seeking to reduce unplanned use of acute and accident and emergency services. Preventive and public health improvement programme: Accelerate Portsmouth healthy town initiative and Southampton healthy cities programme. Innovation spotting and spreading programme: Facilitate the spread of creative ideas across the organisations that make up the partnership.
