Context • Health and Social Care Act (2012) places a duty on Local Authorities and CCGs to produce and publish a Joint Health and Wellbeing strategy • This function to be exercised by the Health and Wellbeing Board (HWB) • The HWB has a duty “to encourage integrated working to advance the health and well-being of people in its area” • A focus on reducing health inequalities and tackling wider determinants of health

  3. Purpose Health and Wellbeing Strategy • Informed by the JSNA • Setting out: • A vision of health and wellbeing for Derby • Health and wellbeing ambitions and priorities • Priority actions

  4. Process • JSNA refresh • Stakeholder Workshops • 173 participants / 36 groups or organisations Including City Council, NHS, Voluntary/Not for Profit • Health and Wellbeing Network • Review and validation of JSNA • Prioritisation and action planning • Partners identified key high impact actions for joint working • Small working group to review and propose high level actions for inclusion in strategy

Vision and aims • The strategy will aim to achieve its vision through: • Promoting good health and wellbeing • Preventing poor health and wellbeing • Managing poor health and wellbeing. “Our goal is to improve the health and wellbeing of the people of the city and to reduce health inequalities.”

More people living longer in better health…our ambition To enable individuals and their families to have the opportunity to have: • The best possible start in life • What they need to develop healthy lifestyles and behaviours • Supportive and safe environments and opportunities for positive life chances.

  9. Better health at work…our ambition To ensure our local working age population are fit and able to work by: • Inspiring our young people to have high aspirations • To prepare our young people and ensure readiness for work • Engaging our employers in promoting health at work • Providing health interventions and lifestyle advice in the workplace.

  10. Better mental health and wellbeing…our ambition To support individuals, families and communities to promote and maintain good emotional, mental health and wellbeing by: • Fostering a positive understanding of mental health issues and their impact • Working holistically, considering an individuals mental and physical health together along with their wider life circumstances

Maternity, Children and Young People- actions Living longer in better health e.g. ready to start school, reducing harm and injuries, healthy lifestyles Better health at work e.g. readiness for work, increasing aspirations, transition arrangements Better mental health and wellbeing e.g. emotional and mental health issues, access to services and psychological therapies

  12. Adults- actions Living longer in better health e.g. Behaviour change programme (b-you), Health and Housing Hub, Rough sleepers Better health at work e.g. Reducing sickness absence, health improvement in workplace Better mental health and wellbeing e.g. Raising understanding and awareness, access to psychological therapies, development of specialist services

Older People - actions Living longer in better health e.g. Integrated care pathway, Rehabilitation and reablement via single point of access Better health at work e.g. improved access to learning and volunteering opportunities Better mental health and wellbeing e.g. dementia strategy and care pathway, networking and social contact opportunities to prevent social isolation
