In the Fit For Life initiative, our goal is to increase colorectal cancer screening completion rates among patients of color by at least 10% in 2011, raising the percentage from 45.7% to 55.7%. The EBAN team has actively engaged with providers and nurses to strengthen CRC screening protocols. Key discussions emphasize tackling disparities through consistent effort and collaboration, including involving Epic specialists to enhance communication about screening needs. We also address the challenge of tracking patients who haven't visited the clinic in the past 18 months.

Progress • What have you accomplished since the last learning session? The EBAN team has met twice with all adult providers and rooming nurses to reinforce CRC screening protocols. • Discuss 1-2 things that you would like to share with others to help them with their work. The disparity for patients of color in CRC screening can be reduced with hard work and diligence. It needs periodic revisiting to keep it fresh. Engage Epic specialists in discussion on how to blow in explanatory CRC screening information into the AVS for patients identified in HM as needing screening.
Questions • What about patients who have not come into the clinic in past 18 months? They are not captured in our numbers.
