Our specialist weight management service is designed for adults over 18 years registered with GP practices in Manchester, Oldham, Salford, Tameside, and Glossop. Eligible patients include those with a BMI of 30+ and in their 12th week of pregnancy or BMI 35+. Through a multidisciplinary assessment, we create personalized health plans combining medical, psychological, and lifestyle assessments. Our program includes group and one-on-one support, continued follow-up, and specialized services tailored to maternal and bariatric needs.

A Specialist Weight Management Service for Manchester, Oldham, Salford and Tameside and Glossop Patients
Eligibility Criteria • Adults over 18 years • Registered with Manchester, Oldham, Salford or Tameside and Glossop GP • BMI 35+ • Ready to change • BMI 30+ and in 12th week or more of pregnancy • GP or health professional referral only (Practice Nurse, Midwife, Consultant, Dietitian)

Group Intervention • An initial two hour multidisciplinary assessment of need • 12 fortnightly sessions over 6 months • Support from a range of specialists in weight management • A further 18 months of follow up sessions, newsletters and continuing support

New Bariatric Criteria • 1st April 2013 • Adults >18 years • BMI 35+ with co-morbidities or 40+ • Obesity present for at least 5 years • GP referral- Minimum 6 months Choose to Change Service • C2C direct referral to provider of choice: Countess of Chester / Salford Royal
