TO EDIT M State of Illinois Benefits Employees newly eligible for benefits have 10 calendar days from their eligibility date to make State plan elections including declining health coverage. • Deadline is in forceregardless of the processing status of job • Effective Date of elections will be either: • Date of hire or employee group change • The date UPB received notification employee has met Substantial Presence (foreign-national visa related).

  3. **IMPORTANT** Faculty: with a 100% appointments that are greater than, or equal to, 4.5 months but less than 9 months are considered part-time for insurance purposes. Insurance Calculation: Length of job/9 x job percentage = insurance part-time percentage Example: 4.5 month job/9 months x 100% = 50% for insurance purposes 50 – 99 % of the average weekly hours required of a full-time employee in the same position Please contact UPB Benefits Services to verify full or part-time benefit eligibility

CLICK TO EDIT M EMPLOYEE’S REPONSIBILITY • Signs up and attend New Hire Benefits Overview Orientation • Makes election for coverage on NESSIE with-in 10-calendar days from hire date for Health/Dental/Life insurance coverage and dependent coverage. • Faxe required documentation to UPB with-in 15 days of hire. • Review enrollment verification from CMS with-in 30 days • Watch for insurance cards should arrive with-in 30 days Note: New Hire’s can review Benefits information prior to coming to campus on NESSIE

  5. TO EDIT M Foreign Nationals Tax Status Review Appointments • All foreign nations must contact UPB to attend a tax status review appointment. • Failure to do so will delay insurance eligibility and the employee will be taxed incorrectly.

CLICK TO EDIT M UPB Benefits Processing • UPB receives FYI/Transaction in HRFE • UPB reviews job to determine benefits eligibility • UPB applies transaction, updated Benefit Category on the job and provides access to New Hire benefits forms. • Access to New hire forms can be provided as part of NESSIE New HIRE and may by-pass UPB.

  7. CLICK TO EDIT M Timing is Everything • Please note the due date for paperwork to be submitted to Human Resources for each pay period. It is extremely important these due dates are adhered to if an employee is to be paid their full pay check on pay day. Any questions regarding the Payroll Schedules should be referred to Payroll Services at: • UIUC: 217-265-6363 • UIC: 312-996-7200 • UIS: 217-206-7211

TO E Default Enrollment • Failure to elect plans within 10 calendar days results in the following default enrollment for both full and part-time employees: • Quality Care Health Plan (QCHP) including EyeMed Vision • Quality Care Dental Plan (QCDP) • Basic Life Insurance • Free 1x annual salary • NO DEPENDENT COVERAGE • Note- a default enrollment will delay employees enrollment into insurance plans, • I. D. cards • Require deduction adjustment to future payroll checks to collect back premiums.

CLICK TO EDIT M State University Retirement System • Participation in SURS is required if you work continuously for at least one academic term or four months, and your employment is not temporary, intermittent or irregular. • You are NOT eligible to be covered by SURS if: • You are a student who regularly attends classes and you are employed on a temporary part-time basis. • You hold a J-1 or F-1 visa and have not established residency. • Retired/Rehired prior to 1/1/2011 and you are currently receiving a retirement annuity.

  12. CLICK TO EDIT M COMPENSATION TAB W-4 Withholding Allowance - Complete or change your State and Federal income tax withholding (W-4) information. If you elect an additional flat amount to be deducted on Form W-4, the flat amount will be deducted on all pay events: original, retro-active, and adjusted. Employee who do not have a W-4 completed on file prior to payroll calculating will have taxes withheld at the highest rate single with zero allowances. Foreign National Employees - Visit the OBFS site to complete and print the Form I9 and schedule a mandatory tax status review appointment with the University Payroll Service Center.

Effective July 1, 2011 University Policy requires direct deposit for all employees.Direct Deposits will be effective immediately. Employees without an active direct deposit account prior to receipt of first pay will receive funds on a university provided Paychek Plus card. • W-2/1042-S Tax Statement - Sign up to receive an electronic version of your W-2 and/or 1042-S, plus view current and historical W-2 information.
