Assessing Incidents-Preventability Scale • Should this have been prevented or avoided? • Rate incidents - Scale of 1 – 5 • 1 Unit / employee are not at fault • 2 Cause was hard to detect • 3 Foreseeable with diligence • 4 Recognizable by a reasonable individual • 5 Obvious hazard or safety procedure ignored

Employee Complaints Investigate promptly Written Plan Must describe how compliance will be achieved Annual review of program Control of Microbial Growth Visual inspections of HVAC and other systems Prompt repair or continue assessment until repair is completed Clean up/drying/removal of wet materials within 48 hours Designate a person responsible for compliance Must be familiar w/ req.
