  2. NRC Findings on Arkansas Nuclear One Accident • On March 31, 2013 a temporary lifting assembly collapsed while moving a 525 ton component (stator)of a turbine, in Unit 1 • Stator fell through hole in floor onto a heavy transport vehicle • One killed, eight injured

  3. NRC Findings on Arkansas Nuclear One Accident • Stator ruptured a water pipe in transport vehicle bay, causing flooding • Certain electrical systems shared by Units 1 and 2 were damaged • Emergency generators used for six days to supply power to cooling systems for Unit 1

  4. NRC Findings on Arkansas Nuclear One Accident • NRC concluded Entergy: • Approved a design for the temporary hoisting assembly that was not supported by detailed drawings, specifications, evaluations and/or certifications • Failed to review associated calculations to ensue that the assembly was designed to support the projected load

  5. NRC Findings on Arkansas Nuclear One Accident • Failed to carry out a load test on the lifting assembly in all configurations for which it would be used • Did not ensure adequate supervisory and management oversight of the contractors and other supplemental personnel involved with the stator lift • September, 2013 OSHA cited 26 safety violations and fines totaling $150,000

Executive Order 13650: Actions to Improve Chemical Facility Safety and Security – A Shared Commitment, May 2014 • Issued August 1, 2013 • Purpose: to enhance the safety and security of chemical facilities and reduce risks associaed with hazardous chemicals to owners and operators, workers and communities.

  8. Executive Order 13650: Actions to Improve Chemical Facility Safety and Security – A Shared Commitment, May 2014 • Directs EPA, DOL, DHS, DOJ, DOT, Dept. Agriculture and DOT to identify ways to improve operational coordination with State, local, tribal, and territorial partners; • To enhance safety and security in chemical facilities; • And to work with stakeholders to identify best practices to reduce safety and security risks in production and storage of potentially harmful chemicals • Modernize polities, regulations and standards

Executive Order 13650: Actions to Improve Chemical Facility Safety and Security – A Shared Commitment, May 2014 • Establishes a Chemical Facility Safety and Security Working Group to oversee effort. • Establishes a Federal Action Plan • Strengthen Community Planning and Preparedness • Enhance Federal Operational Coordination • Improve Data Management • Modernize Policies and Regulations • Incorporate Stakeholder Feedback and Develop Best Practices
