Definition • The Christian Fenger Care Team is a group of support staff who meet every Tuesday to collaborate in finding solutions to the behavioral issues of struggling students. • Our care team is comprised of : • 6 Counselors • 2 Academic Coaches • 1 School Social Worker • 1 School Psychologist • 1 Culture and Climate Coordinator • 2 Mental Health Consultant • 3 External Partners (YES, AOL, BAM) • ,

Goal and Objectives Objectives: Cares about both the individual and the system Assesses by gathering information, discussing and planning Responds by following through with interventions Evaluates the effectiveness of interventions and determines the need to reassess Goal: All students at CFHS will have access to the services and supports they need to feel supported academically, socially and emotionally.

  5. Care team interventions • Individual Counseling Tier 3 • CBITS/ThinkFirst • CBOs • FA/BIP (Special Education) Tier 2 • Boys Town Teaching Model • UMOJA • Hall Sweeps • Peer Jury Tier 1

  6. Referral Sources: PODs, AARs/Hot 75, Referrals from 2009-2010, Administrative requests, Teacher requests, & IES Student does not need intervention at this time. Continue Tier 1 supports. Care Team representative communicates this to referral source RFA &SDQ given to Care Team representative who brings information to Care team meeting to discuss whether student is appropriate for a Tier 2/Tier 3 intervention At Care Team Meeting: Participation in Think First is recommended. At Care team meeting: Participation in CBITS group is recommended. At Care team meeting: Participation in individualized counseling sessions, solution focused brief therapy and/or other intervention is recommended. Case is severe enough to warrant immediate external referral. After care team meeting: CARE Team member completes MSAI and forwards to group leader. Group leader meets with student to determine fit with group After care team meeting: Care team member meets with student to complete the trauma symptoms inventory to determine fit with group. Connect with SASS, or DCFS as needed After Care team meeting: Tier 3 counselors meets with student to conduct assessment Student is Admitted or not admitted into group Not admitted - referred back to CARE Team for reassessment. Consent to participate in intervention is obtained from parent (mandatory for more than 5 meetings) and intervention is initiated. Group co-leader or Tier 3 counselor documents progress notes into Sharepoint system Care Team Referral Flow Chart Following completion of intervention, group co-leaders conduct post-test assessment. Consider impact intervention has had on student and students’ grades, attendance and discipline referrals in particular.
