  3. Self-Determination • Self Determination is a fundamental human right. It means that people, in partnership with family and friends, are empowered to plan and direct how they will live their lives. That people have the freedom to choose the services and supports they need, the authority to control limited resources, and the responsibility for the decisions they make.

  4. Eligibility Criteria • Licensed psychologist, certified school psychologist, • psychiatrist, or licensed physician. • Manifested during the developmental period which is from • birth up to the individual’s 22nd birthday. • Psychological and ABS consist of the following: • The clinical data and an overall score. • A statement by the certifying practitioner that the results are considered valid and consistent with the person’s degree of functional restriction. • A statement by the certifying practitioner as to whether the results indicate that the individual has an intellectual disability.
