This document outlines the collaborative inquiry process undertaken by the Council's Learning & Development Team to prepare staff for the implementation of personalisation and Self-Directed Support (SDS). The key inquiry question focuses on how to provide personalised support that helps individuals achieve meaningful outcomes and empowers them to control their support. The inquiry group, consisting of various stakeholders from social care, has been actively working since December 2012 on action plans, training needs, and ongoing evaluations to improve services and care delivery.
