Aim • Provide a policy and strategic overview affecting the Scottish health and social care system • Explore some of the challenges experienced with the implementation of Scottish policy into nursing practice • Encourage debate and consultation on the direction of travel for community nursing.

  3. Challenges facing Scottish Health and Social Care Systems • Increase efficiency and effectiveness of health and social care • Changes in population demography • Changes in patterns of disease • Changes in lifestyle patterns • Changes in public expectation and demand for quality and personalised care • Inequalities in health status and health care outcomes • Reconciling demand, need and access to health care with safety and quality

  4. Challenging Agenda • Shifting Attitudes • Partnership working • Involvement in design and delivery • Innovation not suffocation

  5. Key action in response to Delivering for Health Acknowledges the health challenges of Scotland’s ageing population Acknowledges the views of Patients and Carers Recommends the merging of disciplines and an end to traditional roles

The New Service Model Individuals, carers, families and communities Maternity Services Acute Sector Community Nurse Consultant Independent Sector care providers Community Hospitals Clinical Team Leader/ Advanced Practitioner Occupational health service providers Local Authority Teams Primary Health Care Team, including practice nurses, community pharmacists etc Community Health Nurse (CHN) Children’s and Young People’s Services Mental Health and Learning Disability Teams Community Staff Nurse Health Care Support Worker Administrative Support Individuals, carers, families and communities

NEW MODEL – CORE ELEMENTS • Working directly with individuals and their carers • Adopting public health approaches to protecting the public • Co-ordinating Services • Supporting Self Care • Multidisciplinary and multi-agency team working • Meeting the health needs of communities • Supporting anticipatory care

Mixed Views of RCN Members • Presents great opportunities for Nurses • Losing identity • What about Children / Young People / Families? • What support will be there for implementation? • Team Leaders / Advanced Practitioners cost money – who will fund? • What training will be available? • Will we still be attached to GP Practices? • Where do Practice Nurses fit?
