  3. Druk op de knop om te startenaanmelden duurt ongeveer 10 minuten

  4. Vul stap 1 in en klik op ‘volgende’

  5. Klik in e-mail op bevestigingslinkVul daarna stap 2,3 en 4 in

  6. Vul stap 2 inklik daarna op ‘volgende’

  7. Vul stap 3 inklik daarna op ‘volgende’

  8. Vul stap 4 in klik daarna op ‘verzenden’

  9. Aanmelding is klaar • Ondervindt u problemen in stap 4 met het invullen van medicijnen, verwijder dan alles en stuur de aanmelding zonder medicijnen. • Vragen? Stuur een e-mail aan lim@lareb.nl • Na aanmelden krijgt u na een aantal dagen vanzelf de eerste vragenlijst toegezonden
