Full Circle – What do we deliver in the Community? • The Full Circle team works with Offenders with Complex and Additional Needs (OCAN) which includes people with • Substance misuse – (68%) • Mental health – (82%) • Learning difficulties – (22%) • The main Criteria is that service users must be at risk of reoffending if those complex needs are not addressed.

Full Circle – What do we deliver in the Community? • Referrals come from a variety sources – mostly via the Courts Essex Health and Justice Service, Custody, Probation Services, Prisons, Housing, Social care, Health services… anywhere! • We support service users to access relevant services – signposting, accompanying to appointments, advocacy. • We have developed close relationships with a large amount of existing community organisations and projects (providing a range of different services) and have set up satellites in these services across the County (we go to where the service user is).

Full Circle – What do we deliver in the Community? • We also… • Work closely with Health & Justice team to identify potential clients for support and court orders such as • Alcohol Treatment Requirements (ATR), Mental Health Treatment Requirements (MHTR) • We act as case coordinators for those who are sentenced to ATRs. This involves initial assessment, timetabling, treatment planning, liaison with CRC/NPS/Treatment agencies. • Prison Link - Ensuring clients recently released from custody enter ongoing treatment in the community.

5. Case study…..‘Stacey’ Stacey is a female in her 30’s with a long history of alcohol misuse. Client was referred to OCAN by the Health and Justice Service after Stacey appeared in court for an assault charge due to her alcohol use Stacey was extremely anxious and vulnerable and so was supported to attend her first appointment with Full Circle by the Health and Justice worker who had assessed her in court. This demonstrates excellent partnership working between the services and enabled the client to ask for the support she so desperately wanted Full Circle were able to support Stacey with Engaging with Alcohol services and accessing the Women’s group to increase her peer support network To register with a GP and sort out getting her correct benefits in place Has been able to remain alcohol free which she attributes to the linked up working and support of services Has been placed on a Catering course by probation This case demonstrates so well how the ‘hand-holding’ approach can work to empower service users to gain confidence and engage with the support that is available.

Full Circle – What do we deliver in HMP Chelmsford? • Full Circle deliver Phoenix Futures’ ‘Building Futures’ model alongside the clinical team (Integrated Drug Treatment System) • Programme is responsive to service users individual need – offers a flexible menu of group modules tailored to the service user. • Interventions offered include: • We also provide 1-2-1 support for service users, work with prison staff to ensure service user safety, and work with community staff to ensure service users from HMP Chelmsford access services on release. • Introduction to Recovery • Alcohol & You • Motivation & Taking Responsibility • Relapse Prevention • Taking Control • Novel Psychoactive Substances • Acupuncture • Yoga • Health & Wellbeing • Mindfulness

7. Full Circle – What do we deliver in HMP Chelmsford? • Care Act 2014 – Full Circle are commissioned to complete Care Act assessments for service users who may require support with daily activities whilst in HMP Chelmsford. Support needs may include - • Physical difficulties • Mental difficulties • Sensory Difficulties • Learning difficulties. • If Care Act need is identified Phoenix Futures will : • Ensure adequate support is put in place for the service user whilst in custody (may include adapted equipment, linking in with prison support available) • Refer in to appropriate support in the community (adult social care, housing etc.) • Ensure Care Act needs are picked up for transfers out & Approved Premises.

8. Background to The Horizon Project • Lankelly Chase Foundation (2015) conducted research in to Severe Multiple Disadvantage (SMD) defined as a combination of: • Substance misuse • Offending • Homelessness, (with poverty and mental health often a factor). • Estimates that in Essex 950 people have contact with all three service areas but individuals often fall through the gaps because services are set up to deal with single issues • The report estimates those experiencing SMD cost public services around £20,000 per year. • Full Circle delivered a 12 month Pilot delivered in Chelmsford working intensively with a cohort of 10 to address the above issues

9. Pilot achievements and outcomes • All 11 clients on the cohort stayed engaged for the full 12 months • We saw a better engagement with services for all clients on the cohort with a wider range of access to services • 8 out of the 11 cohort where housed during ‘The Pilot’ in either temporary, permanent housing or accessed supported mental health housing • There was a marked improvement in self-reported Health and Wellbeing which included a reduction in admissions to mental health units and hospital, and better adherence with medication • We saw a reduction in offending behaviour with no clients self-reporting offending behaviour at the 12 month review. • There was an overall reduction in substance misuse with many clients attending detox programmes and being successfully stabilised on methadone - with some moving from daily supervised to fortnightly or monthly

10. Horizons……… • With funding secured from Essex County Council and TheOffice of the Police, Fire and Crime Commissioner, The Horizon Project is currently being delivered in the following areas:- • Basildon • Chelmsford • Tendring • With further funding from DWP and Mental Health we will soon be going live in the following areas:- • Colchester • Braintree • Epping

11. How does Horizons work………? • Multi-agency approach to identifying the cohort in each area • Agencies work collaboratively and creatively to: • Assertively engage the cohort • Provide support and advocacy • Link individuals to relevant services and support them to attend appointments • Offer an intensive, consistent and persistent approach, offering a positive social relationship. • Offer long term and unwavering support. • Chair multi-disciplinary meetings every 6-8 to ensure a collaborative approach between services.

12. What Horizons have achieved so far…… • Since July 2018 we have engaged with over 40 individuals over the 3 current Horizon Project areas. • 12 individuals have been housed straight from being street homeless. • We have seen some excellent partnership working with PCSO’s, Housing, Probation Services and treatment services……

13. Case study…..‘Emma’ • 37 year old female with long history of very entrenched substance misuse, homelessness and drug related offending. History of poor engagement with services • Horizons has enabled co-ordinated working between Basildon Borough Council, Full Circle, Futures In Mind, Peabody and Probation services and we have been able to: - Support Emma to access her own independent accommodation - Rebuild family relationships and gain the trust of others - Remain substance free since release from custody in August 2018 - Support to access a mentor (FIM) to promote independence - Support to access training and is now working full time Emma has been able to move through the Horizon Project in a positive way, embracing the support available whilst being empowered to make her own choices and decisions…….
