  2. Introduction • Opened on the 31st March 2010 • GP surgery with Walk-In services • Open 8am to 8pm every day of the year • Situated next the Wilson Hospital, Cranmer Road

Why has it opened? • The Wilson Health Centre is part of the NHS Sutton and Merton’s ‘Better Healthcare Closer to Home’ programme (the NHS strategy for improving and modernising how health services are provided in Sutton and Merton) • Helping more local people to obtain the care they need more conveniently • Taking the strain from surrounding hospitals and allowing A&E departments to focus on patients who are critically ill and need urgent care

Who can use the facility? • We provide improved access to treatment for ALL non emergency cases to the surrounding residential and visiting population via a walk-in service • Any resident in the Sutton or Merton boroughs can register with the GP surgery at the Wilson Health Centre • Residents outside the boroughs interested in registering will be reviewed on an individual basis

Walk-In Service • You don't have to register with us to receive care • Opening at 8.00am until 8.00pm every day of the year, our commitment is to provide you fast access to health services and treatment. • The walk-in service provides the same range of clinical care as a traditional doctor's surgery.

Why Register? • There are many advantages in registering with us. • Continuity of care - access to your medical records • Get to know each other – a full new patient health check • Referrals to hospital and other local care - as a registered patient we can refer you to hospital services • Wider choice of appointments availablepre-book appointments so you can see a clinician when it is convenient to you.

Patient Participation Groups • Forum to provide patients the opportunity to feedback on our services • Share opinions and experiences • To encourage involvement in the continual development of our services • To further develop relationships with our patients • Developing a Community Involvement Group for non-patients

Working with the community • We are here to improve everyone's health • In addition to the PCT’s health inequality projects we are proactively working with local organisations to understand more about the local health needs and how we can tailor our service to help meet them • Together, we want to address needs within the community to make the service as flexible as possible for everyone • We want to ensure that every patient, no matter their age, background or personal circumstances, is able to receive good clinical care from our health centre

Examples • Somali Refugee Council – building trust and providing group session advice • Ethnic Minorities Partnership Agency (EMPA) & Merton Involve – presenting at their member meetings and arranging 1-2-1 sessions with member groups. • Educating member organisations on how to use the NHS and the promotion of the NHS translation service through alternative language websites and printed material • Mitcham Festival (Sept) – Health and Well Being Day • Sutton & Merton PCT – The Health Diversity Project • Age Concern – new podiatry service running from 7th July

What can you do? • We aim to raise awareness of our services throughout the Sutton and Merton boroughs using ‘Ambassadors’ • Using the strength of word of mouth • With your work in the community consider the health needs of the groups of people you are working with and consider how we can work together to address them • Partnership marketing – promoting the Health Centre through local businesses and voluntary groups using marketing material
