Downloadable data sets already available on the interim CHDI website Community Health Status Indicators (200+ county-level measures) County Health Rankings (county-level indicators of health and health determinants) Healthy People 2010 (most recent national and state tracking data for HP 2010 objectives) Medicare Compare (quality/patient experience metrics for hospitals, nursing homes, etc.) US Food Environment Atlas (county-level indicators of access to healthy food) National Healthcare Quality and Disparities Reports (state-level indicators) Visit www.hhs.gov/open to learn more -- and download our data! 3

Coming in December 2010: the HHS Health Indicators Warehouse! • New “one stop” shop for national, state, regional, and county-level measures of health, health care performance, and determinants of health • Will be accessible via a public website that will enable users to • -- Explore the data by areas of interest (including condition, geography, population groups) • -- Map, chart, graph, and trend the data (to the extent data allow) • -- Download whatever data they want – and also access the data via web services for easy integration into other websites and applications • Will contain data for all Community Health Status Indicators, County Health Rankings variables, and an array of new and updated measures: • -- Healthy People 2020 indicators • -- State of the USA indicators • -- New Medicare community-level indicators (see next) • -- Plus: evidence-based interventions that have positively impacted performance on indicators 4

New Medicare community-level indicators are coming! • Will debut as part of the HHS Health Indicators Warehouse in December 2010 • Will be aggregate indicators of Medicare service utilization, quality, cost, and prevalence of disease at the national, state, regional, and (potentially) county level • The specific metrics are TBD – we are very interested in getting your input. Possible ideas include: • -- Cost and utilization information such as standardized, risk-adjusted per capita Medicare payments (total and potentially by setting), readmissions, emergency room visits, etc. • -- Prevalence of disease information such as percentage of fee-for-service beneficiaries with various conditions (e.g., congestive heart failure, diabetes) • -- Quality indicator information such as potentially avoidable admissions and the quality metrics found in Hospital Compare 5
