Training Needs Assessment Survey • This will be a web-based survey that will be distributed via e-mail. • I need the names and email addresses of Supervisors within each department to whom I will have this survey sent to. • It will be the responsibility of these Supervisors to then forward the survey to their employees appropriately. • I will need the names and emails of these Supervisors by January 25th.
