  2. Mentor Reporting Mentor This presentation will address our new mentor reporting process. Welcome & Thank You

  3. Mentor reporting consists of two documents, one distributed at the onset of mentoring and one completed at the end • Mentor reporting is not about the case, it's about the volunteer. (No case info is required) • The handout and final report is meant to document expectations and results.

  4. Mentor Activity Plan (Handout) The Mentor Activity Plan is distributed to the new volunteer during their entry level training. This handout should serve as a guideline for their upcoming events. This handout should be consulted at the initial meeting between the new volunteer, their supervisor and mentor to determine how all mentoring activities are to be covered. (A copy of this report is located on the next pages & can be downloaded from the GAL website)

  5. Mentor Activity Plan (Distributed to new volunteer in training)

  6. Mentor Activity Completion Report • This report is completed by the mentor when all mentoring activities are completed and/or discussed. • The completion report contains all activities listed in the handout and reflects how those activities were completed. The underlined area at the end of each section contains one of five possible responses. There is also an optional area for comments at the end. • The completed report should be sent to the volunteer’s respective supervisor and a copy maintained by the mentor. (A copy of this report is located on the next 2 pages & can be downloaded from the GAL website)
