  2. Historia clinica • Paciente: NN • Sexo masculino • Edad: 47 años • Fecha de ingreso: 20/07/12 • Admision Hora: 22:19 • Fecha de egreso: 26/07/12

  3. 20/07/12 • 17:00hs paciente diabetico en uso de lantus 6 UI/dia + glimepirida – conocido HTA en tto con Losartan 50/50mg – no alergico. • Relata que desde la mañana inicio con vertigo + vomitosincohercibes, niega cuadros similares anteriores, afebrilauneico, pulmones limpios a la auscultacion sin quejas alergicas. • Abdomen sp. Diuresis +, sin deposiciones diarreicas. MMII sin edemas

  4. 20/07/12 Diagnostico • - Sx Vertiginoso • - DM tipo 2 • - HTA • - Vomitosincohercibles • PLAN: • - Queda en observacion • - laboratorio • ECG • TAC de craneo y senos paranasales • Reevaluacion

  5. 17:30 hs 20/07/12 • Reposo en cama cabecera elevada • Dieta liviama, hiposodica y para DBT • Sol fisiologico 0.9% 42 gotas por min • HGT preprandial con correccion con IC SC según esquema • B Histina 16 mg – 1 comp VO cada 8 hras • Ondansetron 8 mg 1 amp EV cada 12 hras • Losartan 50 mg 1 comp VO cada 12 hras • Ketorolac 60 mg 1 ampp IV según dolor • CSV y cuidados de enfermeria cada 6 hs • Lab, TAC de craneo, ECG y reevaluar.
