The first is the agency application, the second is the preliminary list of participants to accompany application, and the third is the final participant list due May 1, 2011 • Initial application includes: • Demographic information about the provider • Basic information about proposed participants Must be submitted electronically by: March 18, 2011 To Pilot Workgroup at: DDA45dayPilot@dhmh.state.md.us • Once selected for participation in the pilot : • Submit detailed information for selected participants Must be submitted electronically by: May 1, 2011 to Pilot Workgroup at: DDA45dayPilot@dhmh.state.md.us Note: Applications will be available for submission online only
