Options for Production Reporting • Hardcopy submission • Fill in official form which is available from Forms page link • Postmarked by 3/31 • Form preprinted by DEP with available information pre-populated when address correct in our system. • by early March to: • smaller operators with 1 well – 1,300 operators • 2-20 wells – 1,900 operators • 21-110 wells – 400 operators • > 110 wells - 150 operators - reminder letter only If no letter received by early March – DEP may not have good address. Call Ruth Plant at 717-772-2199 to verify address. OR • File Electronically via web site (preferred)

Well Records - 1 & Completion Reports - 2 • Pursuant to Section 212(b) of the Oil and Gas Act and Section 78.122(a) of Chapter 78 of the Oil and Gas Regulations, a Well Record must be submitted to the Department within thirty (30) days of cessation of drilling or altering a well. • Pursuant to Section 212(b) of the Oil and Gas Act and Section 78.122(b) of Chapter 78 of the Oil and Gas Regulations, a Completion Report must be submitted to the Department within thirty (30) days of completion of the well.
