Synthetisch Cathinone • Jonge bladeren van de plant CathaEdulis (khat) -> 12 typen • Synthetische cathinone uit badzouten bestaan oa uit • 3,4-methylenedioxypyrovalerone (MDPV) • mephedrone (“Drone,” “Meph,” or “MiauwMiauw”) • 18 - 25 euro voor 1 gram • Opsnuiven: effect na 10 tot 20 min, Slikken: na 20 tot 40 min • Sinds 2009 in NL op de markt, hoog risico • onvoldoendebetrouwbareinformatie, verschil van maand tot maand, legaal?

Symptomen • Chemisch gezien zelfde werking als amfetamine en ecstasy • inhibitie van heropname en stimulatie van release van: • norepinefrine (tachycardie, hypertensie) • serotonine ( hallucinaties) • dopamine ( gevoel van euforie en hyperactiviteit) 10x meer ivmcocaine gebruik! • Symptomen vergelijkbaar met amfetamine gebruik (sympathicomimetischtoxidrome) oa. Agitatie, tachycardie, tachypneu, hypertensie, psychose, insulten Ernstige toxische effecten op spieren nog niet eerder beschreven

  5. compartimentsyndroom • pharmacologisch compartiment syndroom ontstaat vaak ten gevolge van directe myotoxiciteit, verhoogde spieractiviteit en /of compressie (langdurige immobilisatie bij IC opname) • Nog niet bekend of synthetische drugs direct myotoxiciteit veroorzaken, echter hyperthermie en verhoogde spieractiviteit wel als oorzaak te noemen. • Behandeling op SEH (zie figuur)

  7. Conclusie • Vele effecten op verschillende orgaansystemen • Bedacht zijn op late symptomen, waaronder rhabdomyolyse/compartimentsyndroom • Multidisciplinaire aanpak
