    1. Sign-off Mentorship

    2. Sign off mentors are essential in supporting and assessing the proficiency of student nurses, for eligible entry onto the register

    3. A mentor Can differentiate between the responsibilities for mentoring a student on final placement Understand principles of due regard One hour protected with the student per week Responsibility for cross referencing previous placements, not to fill in gaps Ability to complete sign-off declaration in respect of previous placement additionally ensuring this standard is maintained on final placement - pass or fail

Importance of signing and dating documentation with the student present Actions to be taken with a failing student Aware on the completion of the 3rd assessment with the student in practice documentation to be completed accordingly and to provide evidence on a yearly basis demonstrating maintenance of skills

Background The sign off mentor role is a requirement of the NMC (2008) Standards to Support Learning and Assessment in Practice Sign off mentors are essential for students in their final practice placement starting from September 2007 cohorts At BHFT staff who qualified as mentors prior to September 2007 will automatically be annotated to the register as a sign off. All other mentors will are required to meet additional criteria

Additional Criteria NMC (2008) state in order for mentors to achieve sign off status they must have been supervised on three occasions for sign off proficiency by an existing sign off mentor Subsequently the NMC approved the use of alternative methods to attain the first and second supervisions for a limited period until incorporated into mentorship modules (Trust Throughout this programme you will complete the first and second assessment in the form of an objective structured clinical examination (OSCE) and to help prepare you for the final supervised sign off in practice

Registered on the same part of the register (Due regard) and working in the same field of practice in which the student intends to qualify Clinical currency and capability, including live status on the mentor register Knowledge of curriculums and the assessment process Accountable to the NMC for decision (pass or fail) Responsible for final decision on students achievement

    8. Role of the Sign Off Mentor To verify that a student has successfully passed or failed to achieve the competencies in the practice placement documents Furthermore to cross reference that all practice placements have been passed and recorded as such on the ongoing record of achievement, Essential Skills Clusters NB: It is not a sign off mentors responsibility to complete empty boxes or re-assess decisions made by previous mentors

NMC Requirements On placement a minimum of 40% of a student time must be spent being supervised (directly or indirectly) by a mentor When in a final placement this 40% is in addition to the protected time (one hour per week) to be spent with a sign off mentor. This protected time allows for reflection, giving feedback and recording of a student progress.

    12. Nurses are individually accountable for their own professional practice. The profession is regulated by the Nursing and Midwifery Council (NMC). The NMC is an organisation set up by Parliament to protect the public by ensuring that nurses, midwives and health visitors provide high standards of care to their patients and clients. It replaces the UKCC The NMC maintains a register of qualified nurses, midwives and health visitors Sets standards for education, practice and conduct Provides advice for nurses, midwives and health visitors Considers allegations of misconduct or unfitness to practise due to ill health Further details of the full range of NMC responsibilities is available at www.nmc-uk.org
