  3. How to Enter a Survey • Go to www.quickbase.com • Click the ‘Sign In’ button in the upper right corner • Enter log-in information: 100KVolunteer@gmail.com • Password: Arlington1

  4. How to Enter a Survey 1) Select ‘Virginia VI’ 2) Click ‘Enter a new survey’

  5. Make sure to select ‘Arlington’ in the Region Name box before entering in data When you are done entering in the data, select ‘Save’ or ‘Save & Add Another’

Eliminating Duplicate Entries • Should only be done after all surveys have been entered • Log-in to www.quickbase.com using your personal account information • From the home page, click the ‘Interviews’ tab in the blue heading • Scroll down in the menu and select ‘Quality Assurance: Duplicate Entries First and Last Name’

  7. Eliminating Duplicate Entries Select ‘Arlington’ from the drop-down menu and press ‘OK’

  8. Eliminating Duplicate Entries Any name with a number higher than 1 in the ‘Number of Interviews’ column potentially has a duplicate entry. Click on the hyperlinked first name to investigate further.

Eliminating Duplicate Entries • 1) Scroll about ¼ of the way down the list until you come to the two sections titled “CI_DOB” and “CI_SSN.” Compare the birthdates and SSN’s from each entry to make sure that they are the same person • 2) If the person has a common name, it will sometimes look as if it is a duplicate entry. Comparing birthdates and SSN’s will ensure that you do not accidentally delete a non-duplicate entry. 4) Select ‘Delete’ on the interview page 3) Click ‘Edit’ next to the interview you want to delete

Community Briefing Report • From the home page, click the ‘Regions’ tab in the blue heading • Scroll down and select ‘Community Briefing Report’ from the drop-down menu You will be presented with a report that has been automatically generated by QuickBase. Included are many data points that will prove useful for a community brief back report.

  11. List of Vulnerable Individuals • From the home page, click the ‘Interviews’ tab in the blue heading • Scroll down and select ‘By Name Vulnerable List’ from the drop-down menu Select ‘Arlington’ from the drop down menu and press ‘OK’

  12. List of Vulnerable Individuals If you would like to export these names (or any report) to an Excel spreadsheet, select ‘Other’ from the menu at the top of the report. Then select ‘Export this Report to a Spreadsheet’ and it will transfer the report to a separate spreadsheet.

  13. Other Customized ReportsList of Vulnerable Veterans • From the home page, click the ‘Interviews’ tab in the blue heading • Select ‘Create a New Report’ from the drop-down menu

  14. Make sure to customize all items outlined in a red box When finished making the customizations, select ‘Display’ or ‘Save and Display’ at bottom of page
