PRIORITY FAMILIES FAMILY SUPPORT WORKER’S Engages with difficult to reach families developing a positive engagement Holds 7-10 cases Provide intensive intervention with a minimum of 2 visits a week (including evenings and weekends where required) Practical support Development of a support plan following a thorough assessment Arranges regular review meetings to measure progress of family Multi agency co-ordination 1:1 and group Triple P support Referrals to other agencies REFERRAL ROUTE Only via Priority Families co-ordinating group No self –referrals accepted Referrals received from police, housing, Children’s Services and education Priority Families work will not run simultaneously to CAF Provide support to 268 families over 2 years CRITERIA One parent in receipt of an Out of Work benefit Education concerns – poor school attendance and exclusions Crime/ASB – reported incidences (2 or 3 of the above criteria required) Housing – tenancy is at risk Alcohol / substance misuse Mental Health Concerns Domestic Abuse Police call outs Fire ASB PRINCIPLES Persistent approach Practical ‘hands-on’ support Dedicated key worker Considering the family as a whole Common Purpose and agreed action

KEY WORKER ROLE Engages with difficult to reach families developing a positive engagement Holds 5-6 cases Provide intensive intervention with a minimum of 3 visits a week, including evenings and weekends Practical support Development of a support plan following a thorough assessment Arranges regular review meetings to measure progress of family Multi agency co-ordination 1:1 and group parenting Referrals to other agencies REFERRAL ROUTE Only via Priority Families co-ordinating group No self –referrals accepted Referrals received from police, housing, Children’s Services and education Family Intervention and CAF do not run simultaneously Provide support to 200 families over 2 years FAMILY INTERVENTION CRITERIA (first three are essential) One parent in receipt of an Out of Work benefit Education concerns – poor school attendance and exclusions Crime/ASB – reported incidences Housing – tenancy is at risk Alcohol / substance misuse Mental Health Concerns Domestic Abuse Police call outs Fire ASB PRINCIPLES Persistent approach Practical ‘hands-on’ support Dedicated Worker Considering the family as a whole Common Purpose and agreed action

FSW ROLE/SUPPORT • Direct work with families in home or community • One to one support with all or part of the family • Practical Support • Triple P Groups/ one to one • Can be lead professional but not key worker • Signposting to other services • Works as part of CAF Family Support Plan. • Caseload of 10-20 families • Planned visits • CAF FSW TEAM • County Wide Team • North Warwickshire – 2 Family Support Workers • Nuneaton and Bedworth – 2 Family Support Workers • Warwick and Stratford – 2 Family Support Workers • Rugby – 3 Family Support Workers • 1 Operations Manager. • 1 Senior Parenting Practitioner • 1 Administration Assistant • Contact on 01926 742194 • FSW CRITERIA • CAF in place and on-going • Voluntary engagement • Early Intervention, Tier 2, Tier 3 and step down from Social care • 6 weekly reviews via Family Support Meeting process • PRINCIPLES • Family wanting support to make changes. • Exploring and encouraging positive parenting • Working alongside families • Evidence based practice • Solution focused work • REFERRAL ROUTE • Referrals via CAF officer as a result of Family Support Meeting. • Completed Referral Form • CAF Assessment in place
