Installation/Implementation • In order for consults to populate into the Authorization module, the services will need to be defined in VistA • A list of all consulting services associated with fee will need to be entered into the “define fee services” option in the Consult Management Menu • Local CAC or ADPAC will have the Consult Management Menu (GMRC MGR)

  6. Purpose • The primary objective ofPatch 23 will provide fee users the ability to create authorizations directly from a consult. • Some consult data will be auto-populated into a newly created 7079 authorization.

  7. Patch 23 • FBCS Authorization will now default to the redesigned home screen upon initial login • The new screen will include three new tables: • Consult table: displaying active and pending consults • Consult details: displaying details of a selected consult • View auths table: displaying previously created authorizations associated within FBCS for the selected consult

Consult Table • In the consults section, a listing of all consults that meet the criteria based on consult status, clinic name, and user based filters will appear in this display • The list displayed can be controlled in the Options 5 menu by users holding the DSIF Administrator key • This information is read-only and cannot be edited • All active and pending consults will show in the consult list unless mappings have been applied.

Consult TableList Date Filter • A date filter appears at the top of the consult table • The default date range is one month prior to the current date • Users can choose a specific date range to retrieve consults • The chosen dates will remain current until the user changes the date range or the active session is terminated

  12. Consult Table The Consults table contains the following data: • Consult Number • Request Date • Patient Name • Patient Initials and last four digits of the patient ID (terminal digit) • Consult Status • Urgency • Consulting Service

Consult Details Table Upon selection: • Users can view the consult details to assist in confirming whether an authorization may be created • Users can view consult data by selecting the record from the Consult Table • When a selected consult is highlighted the “consult details” text box will display the entire consult • You can still copy and paste the selected data

  14. Creating an Authfrom Consult Data Users may utilize the consult to create a corresponding authorization. • This can be accomplished by clicking on the “create auth” button on the right side of the consult list

Creating an Auth from Consult Data • If the consult’s service is rendered on an outpatient basis, the 7079 creation menu will prompt with the consult data entered • If the consult’s service is rendered on an inpatient basis, either the 7078 creation menu will prompt or a clinical tracking record • Supervisor options dictate whether a CT record or notification creation prompt appear

  16. Creating an Auth from Consult Data The 7078 or 7079 is dictated by the type of consult selected. The “Service is to be rendered on Inpatient or Outpatient basis” data line vs the “Current Pat Status” data line will dictate the type of authorization that will be created.

  17. Creating an Auth from Consult Data • The following fields are populated into the 7079: • Patient • From date • Consult ID • Referring Provider • Users can create an inpatient authorization (7078) from the consult, but no data elements are populated.

  18. Ineligible Patients • If the “Create Auth” button is greyed out the patient is not eligible for fee

Removing Finished Consults • Once a consult is ready to be removed from the list, a user can select the “mark as done” button, removing it from the list • The consult status is not changed and it is only hidden from grid. • No data elements are changed within CPRS • Removed consults can be reactivated by a supervisor from the options menu

  20. Patients With Multiple Consults • Consults that are in bold are associated with a patient who has multiple active or pending consults

  21. Authorizations Associated with a Consult • The “Authorizations for this Consult” table will display all authorizations that have been previously created from the selected consult • Only displays authorizations that were created using FBCS to prevent duplicate auths

  23. Print Consult • The “print consult” button will print the consult to the selected VistA printer in the Options menu

Additional Changes • For 7079’s, the “details” and “supplemental data” tabs have been redesigned for the updated required fields • For example, the FCP, Obligation Number, Contract Status, Contract ID, and Vendor have been moved from the “supplemental data” tab to the “details” tab. • Likewise, the DX codes have been moved from the “details” tab to the “supplemental data” tab

  25. Changes to Supervisor Options • New supervisor options have been added to control and manage consults and user mappings • These changes can be found on the “Options 4” tab and the new “Options 5” tab • To access these options, the user must have the DSIF Administrator Key

  26. Reactivate Consults • Allows a supervisor to restore previously removed consults by a user • These consults are “hidden” from users’ consult lists when the “mark as done” button is selected • This option allows a supervisor to “unhide” an active or pending consult for further use

  27. Reactivate Consults • Supervisors must be select which consult needs to be unhidden • Hidden consults will never disappear from this list • To manage the list, a date range filter can be used retrieve only hidden consult for a selected date range

Unlock Locked Consults • When a consult is selected and being worked it is temporarily “locked” to prevent double entry of data from the same consult • If the program crashes while a consult is “locked” it must be manually “unlocked” to reappear in the consult list • Allows a supervisor to “unlock” a consult that was “locked” during a program crash

  30. Inpatient Auth Creation Option • This tool allows the administrator to specify that an inpatient authorization must begin with the creation of a Clinical Tracking record • Selecting the checkbox will default all inpatient authorization creations to prompt into the Clinical tracking record, rather than directly into a notification

  31. Manage User-to-Service Mappings • This button allows the administrator to map specific consult fee services to specific users • Consults for these selected services will only appear in the specified users’ home screen

  32. Manage User-to-Service Mappings • A supervisor must first add a user in order to manage and assign fee services • Clicking on the “Add” button will display a pop-up to search for a user to add • Please note that the username is as displayed in VistA

  33. Manage User-to-Service Mappings • Once the user is added, clicking the “Manage” button will display a source list of fee services • These entries can be added to the selected user’s services list

Manage User-to-Service Mappings • After selecting “ok,” changes will take effect immediately after refreshing - Only the consults with the mapped fee services will appear in the users’ home page • Multiple users can share the same fee services • The absence of these mappings will mean the user will view consults for all services
