  2. Six month intensive outpatient program • Re-entry Transition Services: • Substance Abuse and • Criminal Thinking groups • Transportation • Individual counseling • Employment support • ID/Documentation • Educational/Vocational assistance • Housing Outreach

  3. TheChange…… • Day 1 Peer Specialist picks up client at release. Addresses immediate needs: • Probation • Food • Temporary Housing • Clothing • Bicycle or Bus pass • Day 2 Review welcome packet, recovery meeting, discuss concerns, and enjoy the day! Make plans to meet next day Start one day at time!
