What is Healthy Families? • Comprehensive health, dental, and vision benefits for children (birth to 19) • Low monthly premiums ($4 - $9) • No co-payments for preventive services and $5 for other services Health Plans:Santa Clara Family Health Plan, Blue Cross, Kaiser, Blue Shield, Health Net Dental Plans:Delta Dental, Access Dental, Health Net Dental Vision Plan:Vision Service Plan

  4. What is Healthy Kids? • Created to fill the insurance gaps for kids in Santa Clara County • Healthy Families look-a-like • Benefits • Family contributions / premiums Health Plan:Only Santa Clara Family Health Plan offers Healthy Kids Dental Plan:Delta Dental Vision Plan:Vision Service Plan

  5. What’s Covered? All three programs include: • Full medical, including prescriptions • Inpatient care • Physician visits • Prevention services • Well-baby visits • Health education • Dental care • Vision services • Alcohol and substance abuse care • Mental health

  6. What Qualifies You? • Family Size • Age of Child • Income

  7. Who Qualifies? • Children up to their 19th birthday, *regardless of their immigration status • Families with incomes at or below 300% of the Federal Poverty Level • Children without health insurance • Children who live in Santa Clara County * Healthy Kids

Immigration Status Applying for Medi-Cal or Healthy Families will: • Not hurt your chances to get a green card • Not endanger your current green card • Not prevent you from entering the country so long as you are not gone for more than six months • Not keep you from sponsoring someone to come to the US Source: Immigration & Naturalization Service, May 25, 1999.

What You Will Need to Bring • Medi-Cal & Healthy Families Programs: • Birth Certificate or Immigration papers • Income and/or Deductions statements • Proof of California Residency • If Pregnant: Verification from doctor or clinic • Healthy Kids Program: • Income statements • Proof of that you live in Santa Clara County (Electric Bill, Water Bills, etc.)
