PAFCPIC “WE CARE” REQUIREMENTS FOR Php 100,000 KIA PERSONNEL CLAIMS • NSO Death Certificate of the deceased member or Certified True Copy from the Original issued by City Civil Registrar • NSO Marriage Contract (if Married) • Certificate of Legal Beneficiary from Bn S1 • Affidavit of Undertaking to be issued by the claimants • Certification from Barangay Official (for Associate Member), Unit Commander and /or survivor officer as to the fact of the member’s death • Proof of Coop membership of the deceased member (Original copies of Passbook, Coop ID, Share Certificate/s) Note: • You can claim from any PAFCPIC Branch • All requirements must be “Original Copy”

  2. REQUIREMENTS FOR WIA PERSONNEL CLAIMS (IN-KIND) • Letter from wounded member or letter from Sponsor • Medical Certificate • Quotation of medicines or equipment being requested
