NHS LanarkshireWinter Plan 2009/10 Generic • Monthly meetings in preparing plan • Fortnightly meetings in monitoring/flagging up issues • Daily conference calls which Social Work Department could join as required • Agreed contacts for escalation of issues • Additional funding for SWD activity – e.g. care home placements, additional PH cover • Access to aids/equipment

  3. NHS LanarkshireWinter Plan 2009/10 South Lanarkshire • Reablement team working from Hairmyres (HCSWs) • Routinely 15-20 patients • 30% reduction in home care package • Social Work staff in Public Holidays Christmas and New Year • Impact Christmas low • Impact New Year good
