This guide details the completion of the Child Outcomes Summary Form (COSF) during a child's initial entry and permanent exit from an early intervention program. The COSF includes a cover page and three outcome pages, each with questions designed to evaluate the child's functional skills at entry and exit. Roles on Cover Page • When completing team members’ roles, choose from one of the roles on Table 1 • Choose the role most closely associated with each persons’ role on the team for this child • Data entry into the Outcomes Web System requires choosing from the list of roles on Table 1
Outcomes Pages • Use Table 2 for documenting source of information • Document the who or what of record review, interview, or observation in the summary of relevant results (i.e. SLP observed Jamie in his home, review of Dr. Smith’s records) • Use last date of assessment rather than multiple dates
Summary of Relevant Results • Include positive statements about functional skills observed • Only include information actually used in determining rating • Document what evidence led to the rating • Age expected functioning • Immediate foundational skills • Foundational skills and behaviors that will lead to immediate functional skills
Questions 1b, 2b, 3b on Exit • When child exits from program (Part C or Part B) the rating process and completing COSF process will be repeated • In addition, an additional progress question will be completed for each outcome • If the child gained even one new functional behavior from entry to exit the answer to the progress question is yes and progress is described
