  1. Continuous service improvement: A close focus on hygiene and the control of infectionHeather Whitehead Regional Operations ManagerInitial Hospital Services

  2. Healthcare Commission Standards of particular concern to cleaning services • Core Standards • C4 Health care organisations keep patients, staff and visitors safe by having systems to ensure that a) the risk of health care acquired infection to patients is reduced, with particular emphasis on high standards of hygiene and cleanliness, achieving year-on-year reductions in MRSA; • C21 Health care services are provided in environments which promote effective care and optimise health outcomes by being well designed and well maintained with cleanliness levels in clinical and nonclinical areas that meet the national specification for clean NHS premises.

  3. The Health Act 2006 Code of Practice Duty to provide and maintain a clean and appropriate environment for health care “The environment” means the totality of a patient’s surroundings when in NHS premises. This includes the fabric of the building and related fixtures, fittings and services such as air and water supplies.

  4. Research and Development • Initial Hospital Services are part of Rentokil Initial, a large, international organisation • Parent company who provide financial backing • Rentokil Initial have their own Research and Development department • Initial Hospital Services have a full time dedicated R&D person • We have strong links with research groups in UCLH, Exeter University and other organisations.

  5. Seeking to find a Complete Infection Control Solution Examining and evaluating products and innovations for a wide range of uses: • Surface protection • Air disinfection • Hand hygiene • Laundry • Emergency disinfection

  6. The application of these products, used alongside: • Daily cleaning • Deep cleaning • Training • Monitoring • Management = Complete Infection Control Solution

  7. Innovations considered as being of high interest include • Copper-ion complex • Copper impregnated paint • Surface film impregnated with silver • Chlorine dioxide • Ozone generators for air disinfection and laundry • UV light air disinfection • Alcohol-free hand soaps/gels • Laundry additives Copper door handle UV air cleaners

  8. Copper-ion complex A few copper facts: • Copper pots were used by the Egyptians to store water. • Bordeaux mixture has been used for more than a century in viniculture. • Copper is used in anti-fouling paints. • The human RDA for copper is 1-5 mg/day. • Skin sensitisation to copper is very rare.

  9. Copper-ion complex What is novel about the copper-ion complex? • A charged copper sulphate product developed byRemedy Research. • 20 - 50 times more effective at controlling pathogenic micro-organisms than uncomplexed copper sulphate. • Not an oxidising agent so, unlike many other disinfectants, causes little or no surface damage. • Little health, safety or environmental risk.

  10. Copper-ion complex The product is of particular interest because it has the potential for several applications, including: • A surface biocide applied via microfibre cloth during daily cleaning to maintain a ‘self disinfecting’ surface. UMF fibres treated ... with copper …and with bleach • An alcohol-free anti-bacterial hand gel and/or soap. • A laundry additive to make laundered products ‘self disinfecting’and reduce washing temperature.

  11. Efficacy and Trials • Independent microbiological testing has been carried out at UCLH. The results suggest that the copper complex kills all of the pathogenic bacteria that cause problems in hospitals, including antibiotic resistant clinical isolates. • Trials were carried out with copper impregnated microfibre cloths/mops in a Scottish hospital in February/March 2008. • IHS helped co-ordinate the trial, with an external laundry that washed and re-impregnated the microfibre with copper.

  12. The Scottish results confirmed that copper-impregnated UMF produces a significant reduction in microorganisms. • The small amounts of copper left behind on cleaned surfaces helped to maintain low total viable count. • IHS started a further 3-month trial at one of our southern hospital contracts in June 2008.

  13. All of the work so far indicates that Remedy copper is a product with a lot of promise in the fight against HCAIs • Questions still to be answered: • Can a practical, workable, large scale system be developed? • Will it be a cost effective system? • Will the NHS microbiology and Infection Control specialists be convinced and accept it?

  14. In conclusion… • We believe Initial Hospital Services have much to offer the NHS in contributing to the control of HCAIs and in delivering a clean and safe environment for patients • We are totally committed to our Research and Development work • The work and trials we are carrying out with Remedy copper offers some very exciting prospects for the future
